There has been a lot of Daily 5, CAFE, and Guided Reading talk happening on blogs all summer long. I’m more than a little behind, but I have a short freebie that has served me well.
This sheet works well to record the date, book name, and skill taught to your student/s when you meet with each of them. I have used this exact sheet with success in both Kindergarten and 5th grade. I think I’ll also be using it this fall with my 6th graders. It easily allows me to reference what skills I have taught in the past and what areas I should focus on.

I keep this sheet in a folder. Each student has their own folder that I can easily reference all year, take to meetings about that student, or reference for parent/teacher conferences.
